Subj:   Approval of Advanced Fellowship Program in Patient Safety

  To:    Director (00) and Program Directors, Advanced Fellowship Program in Patient Safety, VA Pittsburgh Health Care System
 

  1. The Office of Academic Affiliations is pleased to inform you that your facility has been approved as a site for the Advanced Fellowship Program in Patient Safety.  You are approved for one fellow each academic year beginning AY 2009-2010.

  2. Details of program administration are found in the Program Announcement that you responded to for this RFP, which can be found on the OAA intranet website at  http://www.va.gov/oaa/specialfellows/programs/SF_patient_safety.asp?p=17. Instructions for the process of nominating candidates to participate in this program are also found at the OAA website, currently located at http://vaww.va.gov/oaa/fellowships/nomination_and_information.asp .

  3. OAA looks forward to working with your site and the National Center for Patient Safety as your site develops your local program.
